Graduation plan Course waiver form This waiver must be signed by the student and parents (or guardian) before a student is considered for registration in a class for which he or she was not recommended. (*Available during registration.)

High School Transcript Release FormThis form is for current students. A transcript release form must be submitted to the student services office before a high school transcript can be sent to colleges and universities.

Physical Education Waiver ApplicationThe graduation requirement of one-half credit of physical education can be waived by completing two seasons of an approved sport, cheerleading or dance team.
